Online Artwork Donation Form
Listed below are the steps in the donation process and a few supporting documents that the Museum requires from the donor.
Submit a digital image of your artwork via this form or printed image by mail, including the title, artist, medium, dimensions, and estimated value.
Or
Arrange a time with the curator to deliver the item(s) to the Phippen Museum and a Temporary Custody Receipt will be issued (This option is chosen so the item(s) can be viewed by the Collection Committee during the next meeting.)

- Provide any information regarding provenance (i.e., when or where the item(s) were purchased, by whom, duration in a previous collection, etc.)
- For donated artwork, the IRS requires a written appraisal for all pieces with a value over $5,000, executed by a qualified appraiser, dated within the last six months. If the artwork is valued under $5,000, a written value from the owner is sufficient.
- Once the image or artwork and details are received, it is presented to the Collection Committee for their consideration and recommendation to the Board of Trustees.
- After a final decision is made by the Board of Trustees, the donor is notified.
